Need:

The Health Equity Project aims to help reduce health disparities for those with pressing social needs such as housing instability, food insecurity, transportation, health system complexity, and other socioeconomic factors by: (1) supporting efforts to connect people from historically disadvantaged populations to needed social services, (2) sharing data among health and social service providers to facilitate improved care, (3) providing population-based data and informatics to analysts, stakeholders, and policymakers, and (4) better engaging with, and reflecting on the views of, community members with lived experience.

CHRT’s role:

The Center for Health and Research Transformation (CHRT) provides backbone support to the local work in Washtenaw and Livingston counties and leads a multi-region learning community to identify ways to share best practices and accelerate progress, and to advance connections with statewide quality initiatives.

CHRT will also provide administrative support to partners in four participating regions across Michigan: (1) the Genesee Community Health Access Program (G-CHAP), administered by the Greater Flint Health Coalition; (2) the Jackson Care Hub, administered by the Jackson Collaborative Network; (3) the MI Community Care (MiCC) program in Washtenaw and Livingston Counties, administered by CHRT on behalf of the Washtenaw Health Initiative; and (4) a community integrated health network, coordinated by HealthNet of West Michigan, with multiple community partners in Kent County.
