Vital Seniors Network Home Nutrition+ Pilot Program
Adults that are 65 years or older need a reliable and consistent source of nutritious foods to live healthy lives, especially if they are living with a chronic health condition. During the COVID-19 pandemic, seniors also need interaction and for community members and health professionals to check in on them regularly. For this pilot, approximately 50 individuals--older adults and adults with disabilities who have diabetes or heart disease --will receive these services for the 12-week pilot duration.
CHRT is providing backbone support to the organizations that are working to advance this pilot. The social service agencies will connect with clients--including referrals from their own agencies and Priority Health clients who qualify for both Medicare and Medicaid--delivering nutritious meals to senior’s homes. Tailored and nutritious menus will be designed by a registered dietitian and meals will be supplied by TRIO Community Meals. In addition, the participating social service agencies will conduct monthly check-ins with clients to make sure that meals were delivered and to address any urgent needs.
