Social determinants of health strategic plan
Many residents in Berrien County face significant barriers to health and well-being due to health-related social needs such as lack of transportation and inadequate housing. These challenges contribute to health disparities and hinder access to essential services. To improve health outcomes, the county seeks a strategic plan to coordinate efforts across health care, social services, and community organizations, ensuring that resources are effectively allocated to address these pressing needs.
CHRT, in partnership with Laurel Health Advisors, is conducting qualitative research and stakeholder engagement to inform the county’s Community Health Improvement Plan (CHIP) and guide the development of sustainable strategies. For this project, CHRT will 1) conduct stakeholder interviews to understand community perspectives on transportation, housing, and social cohesion; 2) analyze findings from interviews to identify gaps, challenges, and opportunities for collaboration; 3) support strategy development by contributing research-based recommendations for the CHIP, aligning with county health priorities; and 4) foster cross-sector partnerships by engaging community leaders, social service organizations, and healthcare providers in the planning process.
By leveraging community input and data-driven insights, CHRT’s work will help create a comprehensive, actionable plan to improve health equity and quality of life for Berrien County residents.
