Vital Seniors Network Home Nutrition+ Pilot Program
Adults that are 65 years or older need a reliable and consistent source of nutritious foods to live healthy lives, especially if they are living with a chronic health condition. During the COVID-19 pandemic, seniors also need interaction and for community members and health professionals to check in on them regularly. For this pilot, approximately 50 individuals–older adults and adults with disabilities who have diabetes or heart disease –will receive these services for the 12-week pilot duration.
CHRT is providing backbone support to the organizations that are working to advance this pilot. The social service agencies will connect with clients–including referrals from their own agencies and Priority Health clients who qualify for both Medicare and Medicaid–delivering nutritious meals to senior’s homes. Tailored and nutritious menus will be designed by a registered dietitian and meals will be supplied by TRIO Community Meals. In addition, the participating social service agencies will conduct monthly check-ins with clients to make sure that meals were delivered and to address any urgent needs.
