Need:

Washtenaw County’s seniors face many challenges on a day-to-day basis–social isolation, food insecurity, transportation needs–and are a rapidly growing population. As of now, the 65+ population in Washtenaw County is projected to more than double by 2045. Collectively, there is a need to transform medical intervention-focused care systems into integrated medical and human service systems and to create accessible resources and options that allow seniors to stay in their homes as long as they are able.

CHRT’s role:

CHRT is working with the Vital Seniors Initiative and the Washtenaw Health Initiative to find places in the senior service system where seniors and their caregivers face barriers; to map the service system’s strengths, gaps, and opportunities for improvement; to gather feedback from focus groups; to use data to publish reports that will help agencies implement changes; to engage in strategic planning processes to create a community action plan; and to support collaborative efforts to develop programs and secure funding for sustainability.
