Support MDHHS in developing possible paths and implement strategic planning.
Ensuring the Level I Assessment tool is best practice and is an effective and efficient standardized tool used across Integrated Care Organizations (ICOs) and their enrollees. The Level I Assessment tool is a survey or questionnaire administered to enrollees in Michigan’s MI Health Link program. It asks a number of questions to determine risk level, additional assessments needed, long-term care services needed as well as personal care preferences.
CHRT will build on CHRT-MDHHS 2019 review of the Level I Assessment Tool for MI Health Link, CHRT will support MDHHS (in using evidence-based research/comparable population programs nationally) to create a standardized Level I Assessment Tool with the greatest potential to withstand programmatic changes in the MI Health Link program moving forward.
Additionally, CHRT will leverage collaborative progress in FY’ 21 to guide review of various LTSS options and the development of an overall LTSS strategy based on landscape research, stakeholder feedback, community input and strategic planning retreats.
CHRT will facilitate discussions, and gather and review feedback from various stakeholders and persons with lived experience.
